A drawing from Alex Okita's sketchpad, likely intended to be used as a fail screen. Although reminiscent of Act 3's fail screen, it went unused.
This piece uses the older face design for Muro which looks more professional and less maniacal. This same design seems to be used for the back of the U.S. box. However in the above art he is not wearing the business suit that he wears in the final fail screen and on the back of the box (the suit which garnered some complaints for clashing with his in-game wardrobe). It also does not appear to be the black jacket which he wears in the actual game, but rather the trench coat seen in the 1999 trailer:
